Modine offers three medical plans through Anthem BlueCross BlueShield, all with prescription coverage through CVS/Caremark: the Blue PPO Plan 1 ($1,500 individual / $3,000 family in-network deductible), the Blue PPO Plan 2 (a lower $750 / $1,500 deductible), and the Blue HDHP 3 ($3,400 / $6,800 deductible) which pairs with a Health Savings Account. All three cover in-network preventive care at 100% and share a $5,500 individual / $11,000 family out-of-pocket maximum.
If you enroll in the Blue HDHP 3 plan, Modine contributes to your Health Savings Account at HSA Bank for 2026: $850 for employee-only coverage and $1,700 for family coverage. Employer contributions are deposited quarterly based on your hire date. You can add your own pre-tax dollars up to the combined IRS maximum of $4,400 (individual) or $8,750 (family), plus an extra $1,000 if you’re age 55+. The balance is always yours and rolls over every year.
Adding a dependent — through marriage, birth, adoption, or guardianship — is a qualifying life event. You have 30 days from the event to record it and complete your enrollment at mymodine.bswift.com, and you must upload supporting documentation (e.g., marriage or birth certificate) to your Bswift Employee File within 45 days. Miss the window and you’ll wait until the next annual enrollment unless another qualifying event occurs.
As you approach Medicare eligibility, Modine provides Allsup Medicare Advisory Services at no cost — licensed advisors who assess your needs, explain enrollment periods, and help you coordinate Medicare with active coverage (available from age 62). Note that at age 65 your Basic Life, AD&D, and voluntary employee life coverage reduce to 65% of the prior amount.
Disability and leave are administered by Lincoln Financial Group. Modine provides Short-Term Disability at 100% employer cost for up to 26 weeks of an approved disability (90-day eligibility; benefits begin Day 1 for hospitalization/surgery, Day 4 for injury or sickness). You may also elect Voluntary Long-Term Disability, which replaces 60% of salary up to $15,000/month after a 90-day elimination period. Report a claim at 888.408.7300 (company code: Modine).
Modine’s 401(k), administered by Principal, offers a Safe Harbor match: 100% of the first 3% of pay you contribute, plus 50% of the next 3%. So if you contribute 6%, Modine adds 4.5%. New hires are auto-enrolled at 3% (auto-escalating 1% per year up to 10%). You’re always 100% vested in your own contributions; the company match vests fully after two years of service.
